Case Management Services

 
What is Case Management?
A Social worker and a Registered Nurse comprise the Team that works closely with the individual, the physician, the family and local Community Service agencies to coordinate services tailored to the individual's needs enabling seniors the choice of remaining in their home.

Case Management Services:

  • Professional assessment of needs
  • Arranging services such as personal care attendants, transportation, homemakers, nutritional services, respite, emergency alert systems, and environmental modification.
  • Monitoring of service delivery and the quality of care
  • Problem solving and clients advocacy.
  • Information and referral.

Who is Eligible?
Case Management Services through Area VI CMT are available for all Senior Citizens within our planning and service area. There are two options for entry, Private Pay or Medicaid.

Private Pay Option
This option is available to all Seniors age 60 and above, who are under the care of a physician or medical practitioner and meet the level of care needed for an intermediate or skilled nursing facility.
Service providers will bill the client or the client's insurance company directly for payment of services as contracted. AREA VI shall bill only for Case Management Services.

Medicaid Option
This option is available to those Seniors aged 65 who meet the eligibility requirements necessary, who are screened eligible by the Foundation, meet the level of care required for an intermediate or skilled nursing facility and are under the care of a physician or medical practitioner.
