Please join Western Montana Area VI to welcome new VDC Idaho Service Coordinator Tawana Swanson!

I have spent my career dedicated to Rural Healthcare access and coordination, focusing on programs that bring independence and choice to my fellow community members.  I have been honored to engage with people centered program development for much of my career.  Over the last 30 years I have been humbled to have held positions such as Quality Review Coordinator (Mental Health).  Case Manager (In-Home Long term care), Administrator (Assisted Living) Development Specialist (Hospital/Assisted Living), Nursing Facility Case Management (State of Washington).  Caregiver (Independent) and Executive Director (Community based organization).

Maintaining in person, one on one connection is my absolute highest motivator, and the gauge used to build person centered action-based plans for Long Term Care Services.  I strive to keep the focus on choice and dignity while maneuvering through complex systems of care.  I look forward to using my skills and knowledge to facilitate our Veterans choices and the growth of our team.
